Why is the audio not properly synched with the video on many internet videos?

Given that computer technology has consistently advanced in recent years, why is it still such a challenge to correctly synch the audio and video on internet videos on sites such as youtube?

For example:

Sometimes, videos seem to start correctly, but end up out of sync. Occasionally, this happens on television, but it seems more prevalent on the web.
